Corewell Health Honored as AMGA’s 2024 Acclaim Award Recipient

Michigan-based Corewell Health has been named AMGA’s 2024 Acclaim Award recipient. The Acclaim Award honors healthcare delivery organizations bringing their organization closer to the ideal medical group and health system as they move toward value-based care. For their accomplishments, Baylor Scott & White Medical Group and St. Elizabeth Physicians were named Acclaim Award honorees.

Alexandria, VA – Michigan-based Corewell Health has been named AMGA’s 2024 Acclaim Award recipient. The Acclaim Award honors healthcare delivery organizations bringing their organization closer to the ideal medical group and health system as they move toward value-based care. For their accomplishments, Baylor Scott & White Medical Group and St. Elizabeth Physicians were named Acclaim Award honorees. 

Structured around the Quadruple Aim, the Acclaim Award criteria include improving patient experience of care, improving health of populations with a focus on quality outcomes, reducing the per capita cost of healthcare, and emphasizing workplace wellness. 

“At AMGA, we believe the criteria of the Acclaim Award is the blueprint for delivering excellent care,” said Jerry Penso, MD, MBA, AMGA president and CEO. “Corewell Health achieved the Quadruple Aim by demonstrating efforts to invest in value-based care by increasing a better care experience for both patients and providers. We congratulate them on their outstanding accomplishments.”

Corewell Health was recognized for the following initiatives:

  • Creating a future where health is simple, affordable, equitable, and exceptional through Safe & Affirming training for team members and the establishment of a navigation hub for LGBTQIA+ patients
  • Implementing Advanced Primary Care (APC), a delivery model specifically tailored for geriatric patients to better meet the needs of a high-risk, high-cost population
  • Developing a transitional care model program (Bridging Older Adults) specifically for frail geriatric patients, using a hands-on approach to ensure this population is well supported through episodes of acute illness
  • Recognizing the influence of quality of each human interaction between patient and care team member on overall experience by utilizing Press-Ganey surveys to measure patient satisfaction
  • Creating the Office of Physician and APP Fulfillment (OPAF), focusing on reducing provider burden without sacrificing quality or patient satisfaction

Baylor Scott & White Medical Group, which is part of Baylor Scott & White Health, was recognized as an Acclaim Award honoree for the following initiatives:

  • Developing an in-house tool to promote the delivery of optimum care, allowing the Operations Improvement team to provide at-the-elbow support to drive performance
  • Taking definitive action to assist Medicaid-eligible patients in reenrolling to restore coverage so they could continue to receive quality care
  • Focusing on proactive and preventive measures to continue its tactic to standardize care, achieving year-over-year improvement in multiple quality scores measured by the Centers for Medicare & Medicaid Services (CMS)
  • Identifying and reducing administrative tasks as a key to reducing burnout, promoting well-being, empowering providers to work at top-of-license, and helping patients see value in the relationship with their provider
  • Reforming provider notetaking to create a new ambulatory visit note template to make a noticeable difference in the well-being of providers

Acclaim Award honoree St. Elizabeth Physicians was recognized for the following initiatives:

  • Focusing on patient experience and engagement through the implementation of a five-year plan, called “Engrained in Excellence,” which included monthly interventions and strategic deployments of resources, as well as monitoring and reporting on patient experience on a weekly basis at the executive leadership level
  • Focusing on quality improvement efforts, such as providing annual wellness exams to over 70% of eligible patients, compared to the national average completion rate of 15%
  • Developing an order for an External Result Request, enabling clinical associates to place an order managed by a centralized Population Health Support Services team, which tracks down and abstracts results for screenings completed in other settings of care
  • Centralizing all emergency department visit follow-up phone calls with members of the care management outreach team in an effort to impact avoidable emergency department visits
  • Providing for and investing in a comprehensive wellness program, composed of four elements—culture development, learning opportunities, resiliency resources, and systemic process improvement—to impact the root causes of provider burnout and unhappiness

AMGA’s prestigious Acclaim Award highlights the continued research and investigation toward finding the finest models of medical management and coordinating care delivery, as well as developing a systemic approach to improving the patient and provider experience.
