AMGA Foundation’s Chronic Care Challenge

The Chronic Care Challenge is the AMGA Foundation’s multi-year initiative to address pressing chronic conditions by harnessing the best practices of AMGA member medical groups and health systems and reversing the staggering burden of chronic disease. Through these campaigns, participating groups address one chronic disease at a time by mobilizing member groups to adopt one or more evidence-based care processes (called “planks”) that lead to measurable improvements. We provide key materials, resources, and guidance to help them achieve clinical goals, and evaluate and measure their progress along the way.

Measure Up/Pressure Down

Measure Up/Pressure Down® (2012-2015)
The first three-year campaign of the Chronic Care Challenge, Measure Up/ Pressure Down® challenged healthcare systems, providers, patients, employers, and the entire nation to improve blood pressure control. Measure Up/Pressure Down® improved hypertension detection and/or control for well over half a million patients and collectively reached more than 177 million Americans through its annual national day of action.

As a natural extension of the campaign, AMGA will offer blinded, comparative benchmarking reports for hypertension control to all AMGA members in 2016. We believe this complimentary offering is particularly valuable and timely as organizations prepare for upcoming payment changes in the Medicare Access & CHIP Reauthorization Act of 2015 (MACRA). Diabetes: Together 2 Goal

Diabetes: Together 2 Goal® (2016-2019)
In March 2016, we launched our second Chronic Care Challenge, Diabetes: Together 2 Goal®, aimed at improving management of Type 2 diabetes in the United States. This three-year initiative will challenge leading healthcare organizations to work together to improve diabetes care for one million people with Type 2 diabetes. Participants will implement one or more “campaign planks” that will improve the care of people with the condition by empowering patients, improving care delivery, and leveraging information technology. Additionally, campaign participants will report data on a quarterly basis to measure progress toward this goal and utilize the campaign’s resources and tools to achieve this mark.

Chronic Care Roundtable
The Chronic Care Roundtable provides an opportunity for industry leaders, national clinical experts, and other key stakeholders to shape future Chronic Care Challenge campaign goals, strategies, tactics, and metrics in partnership with AMGA Foundation.
