Hierarchical Condition Categories (HCCs) provide risk assessments that trigger care coordination and disease-specific interventions. However, accurately determining the current state of HCCs is challenging. Leveraging AI to accurately understand a patient’s disease burden benefits patients, care teams, and improves value-based financial performance. It facilitates effective communication of the patient’s disease status for risk stratification, leading to enhanced care coordination, and improved value performance.
